Public libraries and regional library systems
that wish to be eligible for Year 4 E-Rate funding (July 2001-June
2002) for the telecommunications circuits connecting them to the
ICN should submit a Form 470 ASAP, wait 30 days, then sign the contract
for the circuit. Libraries are encouraged to complete Form 470 online
now, and list the funding year as July 1, 2001 through June 30,
2002.
We have been advised that if a library has
already signed a contract they should still fill out Form 470, wait
30 days, and then request the contracting agent re-do the contract
with dates which accommodate the Form 470 30-day waiting period.
When filling out Form 470, it is suggested
that the library apply for
a) Telecommunications Services (Section 8),
b) Internet Access (Section 9), and
c) Internal Connections (Section 10). The circuit to the ICN will
fall under the Telecommunications Services section. By completing
all three sections, the library will avoid having to resubmit Form
470 if E-Rate funding is desired for other services for Year 4.

To fill out a hard copy, use the attached
Excel spreadsheet (but be advised that the 30-day period will begin
when your Form 470 is received by the Schools and Libraries Division).
When Form 471 becomes available (likely
to be by early October), libraries should list a phone company (Ameritech,
GTE, etc.) as their telecommunications vendor. Do not list the ICN
or ISI.
